Care Management/Care Navigator Services

One of the most important benefits for all AgeWell New York members is having a personal Care Manager or  Care Navigator  assigned to you to help you manage your care. We understand how complex it may be to access the right services, at the right time, and we are here to help. Your Care Manager or Navigator will assist in answering any questions you may have regarding the health care services you may need.

Your Care Manager will work with you to;

  • Seek and coordinate solutions to meet your health and long term care needs.
  • Create an individual care plan that will outline the services that are medically necessary for you to receive.
  • Provide authorization for you to start or continue receiving services, as well as make receiving these services as seamless as possible.
  • Work with your Primary Care Physician and other health care providers to coordinate both your covered and non-covered services.

Contact Hours: Hours are 7 days a week from 8:00 am to 8:00 pm. Note: From April 1 to September 30, we may use alternate technologies on Weekends and Federal holidays.
